Douglas was born Illeana Hesselberg in Quincy, Massachusetts, the daughter of Joan (née Georgescu) and Gregory Hesselberg (aka Douglas). Her paternal grandparents were Rosalind Hightower and actor Melvyn Douglas, and her paternal step-grandmother was actress and politician Helen Gahagan. Her mother is Italian American. She has two brothers, Stefan and Erik.
